Pursuant to Art. 13 section 1 Regulation of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (OJ EU of 4.5.2016 L119) (GDPR) I inform that:

1/ The term User refers to all people who contact us via the forms available on the website, send an e-mail, post a comment, inquiry, click the “like” button, etc. on the Psychological Workshop’s Facebook profile;
and to the Studio’s clients using our services, who leave their personal data in the Studio, such as name and surname, address details, contact details, including: email address, telephone number. Therefore, this data is provided to us voluntarily or automatically.
2/ The term Administrator refers to the Meverse Psychologicy Office, which administers the entrusted personal data, storing it in accordance with the procedures established on the basis of the GDPR Act.
3/ GDPR is Regulation 2016/679 of the European Parliament and of the Council of 26 April 2016 on the protection of natural persons with regard to the processing of personal data, on the free movement of personal data and repealing Directive 95/46/EC.
Who is the Administrator of my personal data?
The administrator of your personal data, i.e. the entity deciding on the purposes and methods of their processing, is

1/ Meverse Psychology Office is the Administrator of your personal data. As a client of the Studio, you entrust us with the following personal data: name, surname, e-mail address, telephone number, PESEL number, address of residence, and in the case of using our website – also the IP address.
2/ Meverse Psychology Office, as the Data Administrator, ensures that it respects the right to privacy of its clients and exercises the utmost care to ensure data security.
3/ Mevers Psychology Office also administers the data of people contacting us via e-mail, Facebook, and contact forms on websites. These data, with the consent of the person contacting us, are added to the mailing database and the customer database and are used for marketing purposes and to answer customer inquiries. Personal data provided in inquiries and forms are treated as confidential and are visible only to authorized persons. When contacting the Meverse Psychological Studio, the Client has the right to request that such data not be included in the above databases.
4/ This Privacy Policy and the Privacy Policy of Facebook apply to data left on Facebook. Meverse Psychology Office recommends that you read these rules and reserves that they may differ from the Administrator’s Privacy Policy.
5/ The processing of your data is necessary to provide the services we provide.
In particular, we use them for:
– implementation of basic services, including:
a/ collecting documentation about the course of consultations, therapy, test results and diagnoses,
b/ issuing opinions, certificates and other documents at your request;
c/ contacting you to arrange appointments, consultations and provide you with other important information, etc.
– handling your requests submitted to us, e.g. via the contact form,
and:
– ensuring payment services;
– debt collection;
– conducting statistical analyses;
– storing data for archival purposes;
– informing about important events and our proposals as part of the information and marketing activities conducted by the Studio.
Due to applicable regulations, data is also processed for tax purposes
and accounting.
6/ Each person who has entrusted us with their data has the right to access, rectify, change or delete the data, as well as the right to limit or deprive the Administrator of the ability to process personal data. If there is concern about a personal data breach, the User has the right to submit a complaint about irregularities to the Inspector General for Personal Data Protection, ul. Stawki 2, 00-193 Warsaw.
7/ Personal data is stored for the entire time necessary to provide the service and for archiving purposes for a period of 10 years from the date of completion of the service. The Administrator reserves the right to process the User’s personal data after discontinuing the provision of the service or withdrawing consent to data processing, for the purposes of archiving medical records, pursuing possible claims, or if national, EU or international law obliges us to retain this data.
§ 3 Access to data
1/ Users’ personal data are stored in electronic and paper form at the Administrator’s office in a way that minimizes access to them by unauthorized persons,
in a secured archive or in encrypted databases and computer systems.
2/ Only authorized persons or external companies cooperating with the Administrator under a contract (e.g. accounting office) have the right to process the User’s personal data.
3/ The Administrator has the right to share the User’s personal data with entities authorized under applicable law (e.g. law enforcement authorities).
4/ The Service Provider does not disclose personal data to entities other than those authorized under applicable law.
